When you are dealing with the VA for home health care options, it is common for the VHA to not explain that if you track hours and activities of daily living yourself, that can then be used as evidence to maintain the current level of care or to request an increase in hours or pay tiers.
This sheet can be used to track hours and tasks done by the:

Caregiver
-
anyone you hire through VD-HCBS (Veteran-Directed Home and Community Based Services) including family members

Anyone who the VA sends to your home as a PCA (Personal Care Assistant)

VA paid home maker
-
VA paid skilled nursing

Companies arranged to supply help will have their own version and you should request a copy or have them fill this out as a duplicate for your records.
By having this evidence you can use this to support a new application, request for additional help, to stay at a specific paid tier through the VA Caregiver program or to justify the monthly amount of VD-HCBS.
By having the Veteran sign as well as the worker/caregiver this becomes legal lay evidence.
